1950's 1960's vintage T. Eaton's Canada Order Form Charge Account Application
I found this while cleaning out an old house that we had bought. I believe it to be from the late 1950's or early 1960's as it was found in a book from that era.
The form is folded and measures 4-1/4 X 2-3/4 inches (10.9 X 7 cm) in it's folded state and measures 8-1/2 X 11 inches (27.8 X 21.5 cm) when unfolded.
One side of the form is an order form that would have been used to order from the Timothy Eaton's Company out of Winnipeg, Manitoba in Canada. The other side of the form is an application for either a Budget Charge Account or a Budget Plan Account.
The T. Eaton Co. Limited was once Canada's largest department store retailer. It was founded in 1869 and closed it's doors due to bankruptcy in 1999. Eaton's competed against stores like Sears and Zellers.

This form is stored in an envelope and it will be shipped in the same envelope placed inside a used greeting card inside of a standard envelope.
